style='letter-spacing: 2px;font-size: 15px;'>入院后立即完善血尿淀粉酶及腹部CT检查，结果提示急性胰腺炎。内科给予胃肠减压、抑制胰液分泌、抑酸、维持内环境稳定、预防感染等治疗。</span></p><p style='line-height: 2em;text-indent: 2.26666em;margin-bottom: 20px;'><span style='letter-spacing: 2px;font-size: 15px;'>入院2天后症状加重，患者出现气短不能平卧，腹痛波及全腹，科室立即复查CT提示胰腺周围渗出增多，大量腹腔积液及双侧胸腔积液。</span><strong><span style='letter-spacing: 2px;font-size: 15px;color: rgb(0, 82, 255);'>血气分析提示呼吸衰竭。经我科会诊考虑重度急性胰腺炎合并ARDS，与家属沟通后转入重症医学科治疗。</span></strong></p><p style='line-height: 2em;text-indent: 2.26666em;margin-bottom: 20px;'><span style='letter-spacing: 2px;font-size: 15px;'>入科后立即行床旁超声检查，并在超声引导下行胸腔穿刺及腹腔穿刺置换引流术，引出血性胸腹水。同时给予CRRT治疗，并请中医科会诊联合中药治疗。转入72小时后给予经鼻空肠营养管置管术，腹胀缓解后并给予肠内营养治疗。同时给予一般治疗:高流量吸氧、抑制胰液分泌、抑酸、镇痛、维持内环境稳定等处理。</span><strong><span style='letter-spacing:
