各设区市教育局，杨凌示范区教育局、西咸新区教育卫体局，韩城市教育局，神木市、府谷县教育和体育局：
根据《陕西省教育厅关于印发陕西省普通高中学生学业水平考试实施意见（试行）的通知》（陕教基〔2008〕18号）精神，为做好我省2021年普通高中学业水平考试理化生实验操作考试工作，现将有关事项通知如下：
一、考试对象和考试时间
1．考试对象：修习完成必修课程的学生；2020年实验操作考试未通过的在校学生及社会考生。
2．考试时间：2021年3、4月。
3．领取登记卡和准考证时间：2021年2月20日至2月28日。
二、考试报名及人数上报
1．报名时间：2020年12月2日—2020年12月8日。
2．准考证号：考生报名时准考证号统一使用省考试管理中心公布的高中学业水平考试考籍号。
3．人数上报：各市（区）按照考试报名人数分县（区）统计，分市（区）汇总，并于2020年12月15日前报送省教育厅教育技术装备管理中心。
三、考试试题
1．考试试题（包括评分卡、评分细则）由省教育厅教育技术装备管理中心组织学科专家，依据课程标准并结合我省实际统
一命制。物理、化学、生物3科各命制6道常规实验操作试题供学生抽签选择。同时，物理、化学、生物3科各提供1道数字化实验操作试题，供有条件的县区选用（数字化实验操作试题选用具体要求另行通知）。
2．各市（区）考试主管部门2021年1月30日后在陕西教育装备网（www.sxjyzb.com.cn）下载考试试题。
3．2021年实验操作考试登记卡、准考证由省教育厅教育技术装备管理中心负责印制，试题、评分卡及评分细则由各市（区）负责印制。
四、考试办法
具体考试办法参照《陕西省教育厅关于印发〈陕西省普通高中理化生实验操作考试实施细则（试行）〉的通知》（陕教基〔2008〕45号）执行。
五、考试工作总结
考试结束后，各市（区）教育局要及时总结全市（区）实验操作考试工作（包括组织领导、具体做法、主要经验、存在问题、改进意见和建议），将工作总结和考试质量分析以书面和电子版
形式报省教育厅教育技术装备管理中心。
六、考试成绩的报送
考试成绩的报送事宜另行通知。
七、考试费用
考试费用按照《关于加强普通高中学业水平考试管理工作的通知》（陕教考〔2011〕1号）执行，不收取任何费用。
请各市（区）按照本通知要求及相关文件精神，认真组织好2021年实验操作考试工作。
