15px;'>食堂从业人员春秋季开学前健康体检“一学期一次”工作普遍落实不到位，多数学校食堂防蝇、防虫、防鼠设施不全；冰箱生熟食品、荤素食品混放现象比较突出；高中、职中食堂外包现象较普遍、食品原材料采购渠道不一；柞水中学学校食堂有6个经营主体，监管难度大；商丹高新学校等多数学校食堂从业人员晨检工作流于形式，安全隐患较大。</span></section><section style='line-height: 2em;'><strong><span style='font-size: 15px;'>二是索证索票及档案管理不规范。</span></strong></section><section style='line-height: 2em;'><span style='font-size: 15px;'>部分学校未留存米、面、油、蛋、奶等原材料供货商的资质证件、供货合同、供货票据等资料；绝大多数学校自行购买蔬菜、调料、半成品食材（手抓饼、速冻饺子）等等无购货票据、无供货人信息；多数学校食品采购清单、质检报告、出入库台账等档案资料留存混乱，未做到及时、分类存档。</span></section><section style='line-height: 2em;'><strong><span style='font-size: 15px;'>三是食品留样不规范。</span></strong></section><section style='line-height: 2em;'><span style='font-size: 15px;'>部分学校食品留样柜卫生不达标，留样信息登记不全，留样量达不到125克；商南县食品留样柜普遍存在未加锁管理，留样柜中有存放其他物品现象。</span></section><section style='line-height: 2em;'><span style='font-size: 15px;'>四是从业人员教育培训管理不到位。食堂从业人员特别是民办学校、民办幼儿园新聘人员食品安全意识较差，食品加工流程、卫生习惯有待进一步提高。商丹高新学校食堂多名从业人员工作期间佩戴耳环、戒指、手镯，有安全隐患。</span></section><section style='line-height: 2em;'><span style='font-size: 2em;'><strong><span style='font-size: 15px;'>一是学校防疫制度落实不到位。</span></strong></section><section style='line-height: 2em;'><span style='font-size: 15px;'>部分学校防疫制度没有落实到日常工作中，个别学校防疫制度、防疫预案与学校实际不符，指导性、操作性不强。学生晨午检、校园日常消毒、因病缺课病因追踪等信息记录不规范。因病复课学生审核程序有漏洞，校医、班主任、宿舍管理员等信息不能互通，未形成闭环。</span></section><section style='line-height: 2em;'><strong><span style='font-size: 15px;'>二是常态化防疫工作有松懈现象。</span></strong></section><section style='line-height: 2em;'><span style='font-size: 15px;'>部分学校对进入校园人员体温检测工作不严格，商南县金丝峡中学设置的防疫隔离点（帐篷）管理不严，环境卫生差，甚至有存放杂物现象。</span></section><section style='line-height: 2em;'><strong><span style='font-size: 15px;'>三是防疫物资管理不规范。</span></strong></section><section style='line-height: 2em;'><span style='font-size: 15px;'>多数学校库存过氧乙酸无生产日期、保质期和失效日期等信息，部分学校库存酒精、过氧乙酸等防疫物资数量较大，存在保质期不能用完造成浪费隐患；多数学校防疫物资日常检查不及时、物资混放现象普遍，安全隐患较大。</span></section><section><br></section><section style='line-height: 2em;'><span style='color: rgb(171, 25, 66);'><strong><span style='font-size: 15px;'>饮用水安全方面</span></strong></span></section><section style='line-height: 2em;'><span style='font-size: 15px;'>一是商丹高新学校等部分具有集中供水条件的学校、幼儿园未接入集中供水管网。</span></section><section style='line-height: 2em;'><span style='font-size: 15px;'>二是使用自备水源的学校水源无防护设施，排污口、溢水口不符合自备水源管理要求，水质检测、水源消毒不及时、不规范。</span></section><section style='line-height: 2em;'><span style='font-size: 15px;'>三是学校净水器、饮水机等水产品日常管理不规范，日常维护、配件更换等信息记录不全。</span></section><section style='line-height: 2em;'><span style='font-size: 15px;'>四是商南县鹿城中学属于二次供水，学校未办理卫生许可证。</span></section><section><br></section><section style='line-height: 2em;'><strong><span style='font-size: 15px;color: rgb(171, 25, 66);'>原因分析</span></strong></section><section style='line-height: 2em;'><span style='font-size: 15px;'>从上述事件和督查情况来看，全市学校安全管理工作存在以下突出问题：</span></section><section style='line-height: 2em;'><span style='font-size: 15px;'>一是对安全管理工作重视程度不够，个别县区、部分学校门禁制度不严，学校学生宿舍、特别是女生宿舍不能实现封闭管理，安全隐患较大。</span></section><section style='line-height: 2em;'><span style='font-size: 15px;'>二是学校食堂管理还不够规范，从业人员安全意识不强，个别学校食堂卫生状况不达标，食品安全隐患较大。</span></section><section style='line-height: 2em;'><span style='font-size: 15px;'>三是常态化防疫工作有松懈思想，防疫制度落实不严格，防疫物资管理不规范。</span></section><section
